Application form must be typed, completed and signed.

An 800 to 1000 word typewritten essay (Arial 12 font) describing your education goals and how those goals will benefit the world around you. Include word count and signature at bottom of page.

Must be a new essay, not used in past applications.

Any special circumstances that would impact you attending college.

Detailed list of extracurricular activities you have been involved in. Address length and efforts of involvement. List all awards or recognitions received and any training received outside of school.

Recommendation from your high school (or foreign equivalent) counselor, teacher or current faculty advisor if already attending college or trade school. ALL recommendations must have current address, phone number and email address of individual writing the recommendation NO EXCEPTIONS!

High School and College (if applicable) transcripts showing grades and grade point average.

National Test Scores, i.e., SAT or ACT, etc (if in college these are used in the evaluation process), or letter of explanation if not available.

A high resolution Head and shoulder 4 X 6 size photo on glossy photo paper, portrait mode, of you alone.
